Q:  How do I get a copy of my shot record, the shot record of my child or a child under my guardianship?

Answer:  E-mail us to request the Shot Record Release Form.  You may also call to request the release form be faxed or mailed.
Note:  The person requesting shot record must be a responsible party on record.

Q: As a licensed daycare facility in Arkansas, how can I access and view the Immunization Network for Children (INC) registry?

Answer: Download and print the following form and return it to the address listed on the form via e-mail, fax or mail. After the Immunization Section receives your application, they will contact you with a user name and password. Once you receive your personal user name and password, the following documents will direct you to accessing the registry and offer a tutorial on locating an immunization record.

Q: As an immunization provider in Arkansas, how can I learn to use the Immunization Network for Children (INC) properly?

Answer:

  • Click on the following link to view the Immunization Network for Children (INC) tutorial. The modules will automatically load, this may take several minutes.
  • Watch one or more modules during a session following the online instructions.
  • Viewing requires Adobe Flash and an internet connection. If you are using a dial up connection, you may not be able to access the site.
  • Local Health Units, private provider offices and anyone responsible for giving multiple immunizations and recording those immunizations in the INC will need to view ALL modules.
  • Day care centers and schools wishing to "view only" records and/or have the ability to print a record will need to view Modules 1,2,3,8,9.
  • If you or your facility are responsible for putting immunizations into INC and keeping your personnel up to date, you will need to view Modules 1-8.
  • There is a test on each module that is printable for your employee file.
  • Your ability to use the information contained in the Modules is limited to your specific access assigned through your user name and password by the Arkansas Department of Health Immunization Network for Children.

Q: Where do I get information on vaccine needed for international travel?

Answer: The Arkansas Department of Health does not provide immunizations for travelers.  Please refer to the list of International Travel Physicians for providers below.

Q: Who is contacted when possible exposure to a communicable disease occurs?

Answer: Contact your local health department.

Q: When are flu shots needed and where do I get them?

Answer: Flu shots are recommended to be given beginning October 15 and are available in local health departments and many physician's offices each year.

Q: How do I report a possible adverse event after receiving an immunization?

Answer: Patients should contact the clinic or provider who gave the immunization.

Exemptions

Q: How do I get a medical exemption for my child to not get an immunization?

Answer: A physician statement outlining medical reasons for an exemption of a specific immunization is to be submitted to the Medical Director of the Communicable Disease/Immunization Section with the exemption form.

The Medical Director will decide whether to grant the medical exemption based on the CDC guidelines.

PLEASE NOTE: Incomplete forms will be returned for completion. A new exemption form must be completed every school year.

Q: How do I get a religious or philosophical exemption for my child to not get any immunizations?

Answer: Religious exemptions can be granted for children "on the grounds that such immunization conflicts with the religious tenets and practices of a recognized church or religious denomination of which said parent or guardian is an adherent or member" in accordance with Arkansas Statute.

Answer: There is a medical exemption for nursing home employees for flu vaccination. A physician statement outlining medical reasons for an exemption of a specific immunization is to be submitted to the Medical Director of the Communicable Disease/Immunization Section with the exemption form.
