Health Connections promotes health literacy, self-health management and the importance of establishing a medical home. Encouraging all pregnant women to receive early and continual prenatal care, connecting Medicaid and ARKids recipients to a primary care physician, and providing Dental Coordinated Care are the major objectives of the program.
Health educators provide education, promotion and outreach, targeting communities with a high Medicaid eligible population. Special emphasis is placed on reaching the target population through schools.
